General Instructions: The Urban Mission asks that all referrals to the Fresh Start program be made by a representative from a social service agency, church, school, health care facility, or individual case manager and not by the client themselves.  

Once a referral form is submitted, an Urban Mission representative will contact the referring agency or the client (if specified) to set up an appointment.  It is strongly recommended that case managers come with clients to this appointment.
